Job Summary

Document patient care activities, ongoing communication, and discharge summaries under the supervision of a licensed physical therapist. Provide physical therapy services including reviewing historical information, administering modalities, and delivering interventions within the treatment plan's limits. Manage an accurate schedule, arrange re-evaluations, and determine equipment needs in collaboration with the supervising therapist. Recognize and document changes in patient condition to contribute to treatment plan modifications. Involve patients and families in treatment and discharge planning through education and training. Pursue continuing education and serve as a clinical instructor to assigned students as appropriate.
