Job Summary
Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist, implementing individualized Plans of Care for patients with orthopedic, neurological, cardiac, and post-surgical conditions. Administer therapeutic exercises, gait training, balance training, and manual therapy techniques while educating patients and caregivers on home exercise programs and fall prevention. Monitor patient response to treatment, report condition changes, and complete accurate electronic documentation in compliance with Medicare regulations. Participate in QAPI and agency performance improvement initiatives while adhering to infection control standards. This Full-Time role offers a flexible daytime schedule with clinical autonomy in the Missouri City, TX area.
Required Qualifications
- Associate degree from an accredited Physical Therapist Assistant (PTA) program
- Active Physical Therapist Assistant (PTA) license in the state of Texas
- Home Health experience (1 year required)
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Current auto insurance
