Job Summary
Facilitate rehabilitation for patients with emotional, physical, or mental disabilities by implementing physical therapy care plans and coordinating care with the home healthcare team. Educate patients and family members about health conditions while preparing timely, complete, and accurate clinical notes. Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members across Northeast, Northwest, Southwest, and Southeast Houston service areas. Full-time and PRN shifts available with competitive pay, mileage reimbursement, and comprehensive benefits.
Required Qualifications
- Assistant Physical Therapist in the state of Texas
- Able to handle physical workload
- Good driving record
- Excellent communication and people skills to effectively interact with patients, families, and healthcare team members
Desired Qualifications
- Home health experience highly preferred
- Knowledge of home health regulations and general healthcare compliance
- Experience with HCHB EMR
