Job Summary
Provide comprehensive skilled physical therapy services in patients' homes under the direction of a Physical Therapist, educate patients and families, ensure safety, and document in accordance with Medicare/Federal guidelines. Requires current PTA license in the state of employment, minimum one year PTA experience in an acute setting, and valid driver’s license with auto insurance. Travel to patient locations is required. Includes a guaranteed base hourly rate with uncapped bonus opportunities and a $5,000 sign-on bonus; Monday–Friday 8:00–5:00 schedule; benefits such as medical/dental/vision, 401k, PTO, and life insurance are offered. Ability to communicate effectively with physicians, therapists, and coworkers; proficiency in documenting clinical notes and completing chart audits; participate in on-call requirements and mandatory trainings. The role focuses on evaluating and treating patients to relieve pain, restore function, and promote independence, within the post-acute care/home health setting.
Required Qualifications
- Physical Therapist Assistant with a current license in the state of employment.
- Minimum of one year of experience as a PTA in an acute setting is required.
- Home care experience is desired.
- Valid driver’s license and auto insurance with your name as a listed driver.
