Job Summary
Provide in-home Physical Therapist Assistant (PTA) services under the supervision of a Registered Physical Therapist, delivering hands-on therapeutic interventions across neuromusculoskeletal, pulmonary, and cardiovascular conditions in patients' homes and community settings. Deliver and modify patient care plans, educate patients and families on home exercise programs and procedures, document progress per regulatory standards, and ensure patient safety while coordinating with the care team to support ongoing rehabilitation and independence.
Required Qualifications
- Graduate of a state-approved Physical Therapist Assistant program
- One year of experience under appropriate supervision; or a combination of education and experience demonstrating the necessary competencies for the position
- Current New Hampshire license to practice as a Physical Therapist Assistant
- Ability to utilize a portable electronic field device and manage ePHI remotely in accordance with HCS policies
- Valid driver's license and access to a vehicle meeting NH registration and auto liability insurance requirements
