Job Summary

Under supervision of a licensed Physical Therapist and Clinical Manager, implement medically prescribed physical therapy programs including modalities, exercises, and teaching to relieve pain and restore physical function. Coordinate rehabilitation services for PACE Southeast Michigan participants within the interdisciplinary team, assisting with 24-hour care needs, discharge planning, and level-of-care transitions. Provide treatments, advance care plans, and document all interactions in the Electronic Medical Record. Assist with initial and periodic assessments, manage Durable Medical Equipment, and delegate activities to support staff. Maintain clinical productivity standards, ensure participant safety, and participate in professional development and in-service education. Requires current MI license, APTA-accredited degree, and one year of experience with frail or elderly populations.

Required Qualifications

  • Associates degree as a Physical Therapist Assistant from a school accredited by the APTA
  • Current state of MI license
  • One year (1) minimum experience with a frail or elderly population
  • Physical ability to constantly exercise, lift patients, push wheelchairs, and move equipment
  • Current CPR for Healthcare Providers
