Physical Therapist Assistant-Outpatient At Home Per Diem (average 8-10hrs/wk)

Physical Therapist Assistant (PTA) needed for outpatient at-home care, per diem, averaging 8-10 hours per week. Under the supervision of a licensed Physical Therapist, implement individualized treatment plans, provide PT treatments, assist with tests/evaluations, and document care in the EMR. Coordinate with the PT and rehabilitation team to meet short- and long-term goals, maintain professional documentation, participate in meetings, ensure safety and confidentiality, and support quality improvement initiatives in Hebrew SeniorLife's therapy programs.
