Job Summary
Conduct physical therapy interventions and patient/caregiver education under Physical Therapist supervision, modifying treatment procedures per the plan of care and NY State Education law. Monitor patient progress through observation and follow up with the supervising therapist, while managing assigned caseloads and delivering direct care to patients in homes, care facilities, and other environments. Assist with therapeutic activities, positioning, transfers, and lifting using durable medical equipment, and transport required technology devices and supplies to and from patient locations. Document all care provided and maintain records per VNS Health policy, participate in interdisciplinary team meetings and caseload reviews, and comply with professional practice standards.
Required Qualifications
- New York State Certification as Physical Therapist Assistant
- Valid driver's license
- Associate's Degree as Physical Therapist Assistant from a New York State approved program or whose educational credentials have been approved equivalent by New York State
- Minimum of two years experience in clinical patient care
