Job Summary
Provide exceptional patient care to help individuals regain strength, mobility, and independence within a supportive skilled nursing environment. Assist licensed therapists in developing and implementing individualized treatment plans to address patient-specific goals. Maintain accurate documentation of patient progress and communicate regularly with the care team. This role requires an active Illinois PTA license and graduation from an accredited program. Required Qualifications
