Job Summary
Perform treatment, provide exercise instruction, and deliver patient education aimed at improving well-being while following the established plan of care. Observe, record, and report patient responses to the supervising PT, nurse supervisor, and/or physician. Instruct patients, families, caregivers, and the healthcare team on therapy procedures within your scope. Coordinate care with other team members and make recommendations for changes in the plan of care. Adhere to state-specific laws governing home care physical therapy and perform only procedures you are qualified and trained to execute. Available to work a weekend rotation.
Required Qualifications
- Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
- Available to work a weekend rotation
- Candidates are required to pass a drug test before beginning employment

- Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility
