Physical Therapist Assistant- Home Health, Per Diem

Provide physical therapy services to patients under the supervision of a registered Physical Therapist, treating them within a plan of care to relieve pain, restore function, and maintain performance using exercise, massage, heat, and electrical modalities. Direct patients in active and passive exercises, muscle re-education, gait training, and activities of daily living, including transfer and prosthetic training. Observe, record, and report patient reactions and condition changes to the RPT while teaching self-care techniques. Complete and submit accurate clinical notes and documentation in a timely manner. This per diem role is based in the Upper Valley surrounding area with a focus on home health and hospice services.
