Physical Therapist Assistant, Home Health, Full Time

Provides physical therapy treatments under the supervision of licensed physical therapists, including modalities, manual techniques and common exercise techniques. Instructs and guides patients through functional activities such as bed mobility, transfers, gait and balance activities. Works under minimal supervision and may mentor and train staff on technical skills. In the home health/hospice setting, delivers care across communities with a focus on in-home therapy; performs timely documentation of interventions, and offers education to patients and caregivers while adapting to learning styles. Ensures patient safety and follows treatment plans, reporting changes to supervising therapists and nurses. Related duties include chart documentation, caregiver education, and participating in ongoing evaluation of therapy needs.
