Physical Therapist Assistant, Home Care, Bronx

Deliver physical therapy interventions to designated patients under the supervision of a Physical Therapist, including positioning, moving, transferring, and lifting patients while instructing family members on durable medical equipment use. Monitor patient progress through observations and follow up with the supervising therapist, modifying treatment procedures per the PT plan of care and documenting all care provided. Travel to patients' homes and care facilities to provide direct care, utilizing required technology devices and infection control supplies. Manage assigned caseloads and participate in interdisciplinary team meetings, in-service training, and caseload reviews. Comply with NY State Education law and VNS Health policies, including co-visits with Physical Therapists for all initial visits.
