Job Summary
Under the direction of a Registered Physical Therapist, provide patient services, assist in evaluation and development of a rehabilitative plan of care, and participate in the preparation of clinical and daily progress notes. Assist in periodic re-evaluation as required and assume other duties deemed appropriate by the PT, Registered Nurse Case Management, and Nursing Supervisor. This role involves driving to access patients' homes within a 30-mile radius of Centerville, with the ability to navigate non-routinely wheelchair accessible locations. Perform physical assessments requiring sufficient hearing, eyesight, and dexterity, including lifting and carrying up to 30 pounds and moving patient weight up to 250 pounds. Use the Roger AI application to record notes and assessments, allowing for faster EHR chart completion. Work Monday through Friday, choosing your own hours.
Required Qualifications
- Must be a graduate of an approved school giving a course of not less than two years for Physical Therapy Assistants
- Must be licensed by the State as a Physical Therapy Assistant
- Must have a criminal background check
- Must have current CPR (BLS) certification
- Ability to drive
- Ability to access patients' homes which may not be routinely wheelchair accessible
- Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care
- Must be able to bend, stoop, reach, and move patient weight up to 250 pounds
- Must be able to lift and/or carry up to 30 pounds
