Physical Therapist Assistant / FT

Carry out Care Plans designed by the Physical Therapist, assist during evaluations, and monitor resident progress while communicating updates. Perform resident home assessments as part of the discharge process and conduct admission screenings, along with quarterly and annual screenings to determine therapy needs. Maintain documentation of resident activities, record progress, and handle billing activities in the system. Comply with all applicable rules, policies, and guidelines related to employment with Westminster Services. This role requires direct supervision from the Out-Patient Therapy Director or Therapist, with some travel between communities required.
