Physical Therapist Assistant

Deliver physical therapy interventions to designated patients under the supervision of a Physical Therapist, including assisting with positioning, transferring, and lifting patients while instructing family members on therapeutic activities. Manage assigned caseloads by following established plans of care, modifying procedures as needed, and documenting all care provided per VNS Health policy. Travel to patient homes and care facilities using approved transportation to provide direct care, transporting required technology devices and supplies. Monitor patient progress through observations and follow up with the supervising therapist, ensuring compliance with NY State Education law and VNS Health standards. Participate in interdisciplinary team meetings, in-service training, and caseload reviews.
