Job Summary
Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist; carry out treatment plans to help patients regain mobility, function, and independence; communicate progress to the supervising therapist; educate patients and caregivers on exercises and safety practices; document visits and progress notes accurately; participate in care team meetings and interdisciplinary planning; uphold infection control and safety standards during all home visits; maintain professional rapport with patients and families; serving Truth or Consequences, NM with flexible scheduling and autonomy.
Required Qualifications
- Current licensure as a Physical Therapist Assistant in New Mexico
- Reliable transportation and valid driver\'s license
- Strong interpersonal and communication skills
