Job Summary
Deliver individualized treatment plans under the supervision of a Physical Therapist in in-home and senior living settings. Provide therapeutic exercises and functional activities, document visits in FOX’s EMR, collaborate with an interdisciplinary team, and contribute to patient independence and mobility within the Geriatric House Calls model.
Required Qualifications
- Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain
- Current CPR certification
- Basic computer skills; proficient in Microsoft Office
- ability to meet physical demands of in-home and community-based care (lifting, transfers, mobility)
- Graduate of an accredited Physical Therapist Assistant program
