Job Summary
Perform treatment, provide exercise instruction, and deliver patient education aimed at improving well-being while following the established plan of care. Observe, record, and report patient responses to treatment and condition changes to the supervising PT, nurse supervisor, and physician. Instruct patients, families, caregivers, and healthcare team members on therapy within the scope of practice. Coordinate care with other healthcare team members as appropriate. Adhere to state-specific laws governing physical therapy in home care, performing only procedures qualified and trained to execute. Work a weekend rotation with full-time hours (40 weekly).
Required Qualifications
- Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
- Available to work a weekend rotation
