Job Summary
Provide skilled physical therapy treatment in accordance with established plans of care for adult and geriatric patients in a skilled nursing facility. Implement individualized treatment programs focused on functional outcomes and monitor patient response to communicate progress with the supervising Physical Therapist. Maintain timely, accurate documentation to support quality care and reimbursement standards while collaborating with interdisciplinary team members. Requires a graduate of an accredited Physical Therapy Assistant program with current licensure and experience in skilled nursing or post-acute rehabilitation.
Required Qualifications
- Graduate of an accredited Physical Therapy Assistant program
- Current Physical Therapy Assistant license (or eligibility for licensure) in applicable state
- Strong communication, organization, and clinical reasoning skills
- Commitment to patient-centered care and professional standards
Desired Qualifications
