Physical Therapist Assistant (Beaver Dam)

Job Summary

Provide physical therapy interventions to patients under Physical Therapist supervision, ensuring treatments are safe, timely, and patient-centered. Manage technical, environmental, and financial resources while documenting care in accordance with regulatory guidelines. Collaborate on treatment plans, referrals, and infection control protocols, and contribute to safety process improvements. Complete required education, training, and competency assessments. Maintain a Wisconsin driver's license, professional licensure, and CPR/BLS certification. Work flexible schedules including weekends and holidays, with a minimum of 0.5 FTE or one shift per month for per-diem roles.

Required Qualifications

  • State of Wisconsin Driver's License
  • State of Wisconsin Professional licensure for Physical Therapist Assistant
  • Associate degree in Physical Therapist Assistant
  • A minimum of one (1) year of recent experience as a licensed Physical Therapist Assistant
  • Familiarity with patient care processes and equipment specific to the PTA role
  • CPR/BLS certification
  • Valid Auto Insurance
  • Reliable vehicle transportation
  • Work schedules determined to meet patient care needs
  • Work may be required on weekends and holidays
  • Minimum scheduled role is 0.5 FTE
  • Minimum work expectation for a per-diem role is one (1) shift/month
  • Under the direction and supervision of the Physical Therapist
  • In compliance with organizational policies and procedures
  • Consistently with regulatory guidelines
  • PTA's competency
  • General and direct supervision
  • Documentation in accordance with professional and regulatory guidelines
  • Understand, demonstrate, and follow organizational infection control guidelines (CDIC)
  • Promotes a culture of safety by taking accountability for individual actions
  • Promptly reporting errors or near misses via the organization's occurrence reporting system
  • Participating in developing improved safety processes
