Job Summary
Provide one-on-one physical therapy to older adults in-home or in senior living communities, including evaluations, development of individualized plans of care, and evidence-based treatment to improve mobility, independence, and aging-in-place outcomes. Deliver in-home and community-based therapies, document effectively in the EMR, collaborate with an interdisciplinary team (PT/OT/SLP/Fitness Specialists), educate patients, caregivers, and staff, and offer telehealth/virtual care when appropriate to extend access. Must maintain a valid PT license, have a degree from an accredited PT program, CPR certification, and be capable of meeting physical demands of in-home care. H1B sponsorship available for qualified clinicians. New graduates may participate in mentoring programs. Required Qualifications
- Valid Physical Therapy license in the state(s) of practice
- Degree from an accredited Physical Therapy program
