Job Summary

Join AdvisaCare as a Home Health Physical Therapist (PT) or Physical Therapist Assistant (PTA) providing one-on-one physical therapy in patients’ homes across Detroit, Wayne County, Washtenaw County, and Macomb County, MI. Responsibilities include conducting comprehensive PT evaluations, developing and implementing individualized treatment plans, delivering evidence-based interventions in accordance with physician orders and CMS Conditions of Participation, maintaining accurate documentation, collaborating with patients, caregivers, physicians, and the care team, educating patients on therapeutic exercises, gait training, transfer techniques, fall prevention, and Home Exercise Programs (HEPs), reassessing progress, and communicating changes to the care team. Qualifications emphasize a PT/PTA degree, an active Michigan license, home health experience (preferred), strong clinical judgment, effective communication, independence with teamwork, and reliable transportation with a valid license and insurance. Required Qualifications

  • Degree from an accredited Physical Therapy (PT) or Physical Therapist Assistant (PTA) program.
  • Current Michigan Physical Therapist/Physical Therapist Assistant license in good standing.
  • Home health experience preferred (minimum one year).
  • Strong clinical judgment, critical thinking, and problem-solving skills.
  • Excellent communication, interpersonal, and patient education skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Reliable transportation, a valid driver's license, and current auto insurance.
