Job Summary
The Physical Therapist (PT) provides comprehensive skilled therapy services in patients' homes as ordered by the attending physician, including conducting OASIS assessments, developing and modifying the plan of care, and educating patients and families. Responsibilities include treating patients to relieve pain and restore function using physical therapy modalities (exercise, massage, heat, etc.), coordinating services for outpatient needs when home care cannot be provided, ensuring timely and accurate documentation per Medicare/Federal guidelines, supervising PTAs and aides as appropriate, maintaining licensure and driving to assigned patient locations, participating in on-call requirements, and contributing to a patient-centered care transition from facility to home. The role emphasizes safety, patient and family involvement in care planning, and adherence to agency policies while supporting post-acute, home-based rehabilitation.
Required Qualifications
- Current Physical Therapist license in the state of employment
- Minimum of one year of experience as a PT in an acute setting (home health experience desired)
- Ability to drive and travel between patient homes as scheduled
- Effective communication with patients, families, physicians, and co-workers
- Knowledge of Medicare/Federal guidelines and agency protocols
- Ability to develop and adjust plan of care and document accordingly
