Job Summary
The Physical Therapist delivers skilled PT services in patients’ homes under physician orders, educates patients and families, completes accurate documentation, and complies with legal/Medicare guidelines. Responsibilities include conducting OASIS assessments to develop and adjust plans of care, treating to relieve pain and restore function, coordinating with care teams, supervising PTAs and aides when appropriate, ensuring DME and equipment use is understood, driving to patient locations, and maintaining professional licensure and ongoing training. The role emphasizes patient and family involvement in care planning, proper documentation within agency protocols, and contributing to reducing hospital readmissions through preventative education.
Required Qualifications
- Minimum of one year of experience as a Physical Therapist in an acute setting is required.
- Home health experience preferred but not required.
- Active Physical Therapist license in the state of employment.
