Job Summary
Provide comprehensive skilled physical therapy in patients' homes as ordered by the attending physician, including performing OASIS assessments, developing and adjusting the plan of care, educating patients and families, and ensuring safe patient care and accurate documentation. Requires active PT license in the state of employment, at least one year of acute-care PT experience (home care experience desirable), ability to travel to patients' homes, and strong communication with patients, families, physicians, and co-workers; supervision of PTAs/nurse aides as applicable; adherence to Medicare and agency guidelines; and active documentation within required timelines.
Required Qualifications
- Physical Therapist with a current license in the state of employment
- Minimum of one year of experience as a PT in an acute setting
- Home care experience is desired
- Valid driver’s license and auto insurance with your name as a listed driver
