Job Summary
Provide comprehensive skilled physical therapy in a patient's home as ordered by the attending physician, including pain relief, functional restoration, and equipment instruction. Develop individualized plans of care through OASIS assessments, adjust treatments based on patient response, and counsel families on disease management and preventative measures. Supervise therapy services for patient assistants and nurse assistants while ensuring accurate clinical documentation, chart audits, and timely submission of all visit records within 24 hours. Maintain active licensure, complete mandatory trainings, and participate in on-call rotations and case conferences. Drive to assigned locations in Lafayette, IN, and surrounding areas, adhering to Monday-Friday 8a-5p schedules and agency policies.
Required Qualifications
- Minimum of one year of experience as an PT in an acute setting
- Physical Therapist with a current license in the state of employment
- Valid driver's license
- Auto insurance with your name as a listed driver
