Job Summary
Provide skilled physical therapy in patients’ homes as ordered by the attending physician; develop and implement a plan of care; perform OASIS assessments, document all treatment and progress, supervise PTAs as applicable, and communicate with patients, families, physicians, and agency staff. Travel to patient homes in the Fairfax, VA area is required; on-call responsibilities and in-home visits are part of the role. Maintain active PT license, meet health requirements, and adhere to Medicare/federal guidelines. The role includes driving, scheduling flexibility, documentation excellence, and collaboration with a multi-disciplinary team to reduce hospital readmissions and improve patient outcomes.
Required Qualifications
- Physical Therapist with a current license in the state of employment
- Minimum of one year of experience as a PT in an acute setting is required
Desired Qualifications
- Home care experience preferred
- OASIS experience preferred
- Experience supervising PTAs
- Ability to drive to patient homes
- Flexible schedule
