Job Summary
Physical Therapist I providing skilled therapy at patients’ homes under physician orders. Delivers comprehensive PT services, conducts OASIS assessments, develops and adjusts individualized plans of care, educates patients and families, ensures safety and high-quality documentation, and coordinates with PTAs and home health staff. Drives to multiple assigned home locations within Michigan; promotes continuity of care, reduces rehospitalizations through preventative planning, and participates in on-call and mandatory meetings. Requires an active PT license in the state of employment, a valid driver’s license, and at least one year of acute-care PT experience; sign-on bonus of $15,000.00 offered.
Required Qualifications
- Active Physical Therapist license in state of employment
- Valid driver's license and auto insurance with your name as listed driver
- Minimum of one year of experience as a PT in an acute setting is required
- Ability to communicate effectively with patients, families, physicians, and coworkers
- Knowledgeable on therapy best practices
- Detail-oriented, disciplined, and timely
- Ability to evaluate and adjust plan of care based on patient response
- Proficient documentation in accordance with Medicare/agency guidelines
