Job Summary
Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders. Perform start of care evaluations, resumption of care, recertifications, and discharges. Assess patient needs, plan interventions, and modify care to meet goals while collaborating with the interdisciplinary team. Educate patients, families, and caregivers on home exercise regimens. Requires a valid California Physical Therapy license, current CPR certification, and willingness to travel to patient homes. Submit your most current resume to apply. Required Qualifications
- Active license/registration with the Physical Therapy Board of California
- Graduate of an accredited Physical Therapy program
- Strong documentation and organizational skills
- Excellent communication and interpersonal abilities
- Willingness to travel to patient homes for evaluations and treatment
- Flexibility regarding work locations throughout our service area
- Valid California Physical Therapy license
- Current CPR certification
- Valid California driver's license
- Current California auto insurance
- Completed physical examination, TB test, and required vaccines/titers
- Two professional references
