Job Summary
Provides physical therapy services to patients in their homes under a physician's plan of care; develops and implements effective treatment plans; translates exercises into functional activities or activities of daily living; supervises physical therapy assistants; maintains clinical records and progress notes; educates families and coordinates with the provider; travels to Grayslake, IL area for home health PT services; covers surrounding areas as needed; emphasizes improving mobility and overall quality of life.
Required Qualifications
- Graduate from a School of Physical Therapy approved by the American Physical Therapy Association or Allied Health Education and Accreditation of the American Medical Association and the Physical Therapy Association
- Current licensure as a Physical Therapist in the state of practice
- At least two (2) years’ experience in an acute hospital or equivalent experience (preferred)
