Job Summary
Physical Therapist (PT) – Full-Time Home Health position in Camden, TN and surrounding counties. Provide physical therapy services per physician plan of care and state guidelines; evaluate patient function and help develop individualized care plans; educate patients, families, and caregivers on treatment plans and therapeutic activities; translate prescribed exercises into functional activities and activities of daily living; recommend home modifications to support safety and independence; collaborate with physicians and the interdisciplinary team to monitor progress; document clinical notes and progress reports; supervise Physical Therapist Assistants; maintain accurate records and track patient goals/outcomes; mileage reimbursement is offered as part of the benefits package.
Required Qualifications
- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
- Current Physical Therapist (PT) license in the state of practice
- At least two (2) years of experience in an acute care or similar setting (preferred)
- Current CPR and First Aid certification
- Reliable transportation, valid driver�s license, and current automobile insurance
