Job Summary
Provides physical therapy services to home health patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act. Develops and implements effective treatment plans, translates exercises into functional activities or activities of daily living, and participates in the development of the plan of care. Supervises PT assistants, maintains accurate clinical records and progress notes, and communicates with the attending provider about patient progress and environmental factors. Educates and supports families to motivate patient progress, and prepares clinical and progress summaries based on goal attainment. Works in patients' homes in Joliet and surrounding areas, delivering in-home physical therapy services and contributing to patient mobility and quality of life.
Required Qualifications
- Graduate from a School of Physical Therapy approved by the American Physical Therapy Association or Allied Health Education and Accreditation of the AMA and the Physical Therapy Association
- Current licensure as a Physical Therapist in the state of practice
