Job Summary
Physical Therapist provides and coordinates PT services for PACE participants as part of the interdisciplinary team, including in-person assessments at enrollment and ongoing, planning and implementing treatment programs, education for participants/family/caregivers, training for PACE staff on body mechanics and safety, and collaboration to develop comprehensive care plans and 24-hour care delivery aligned with participant plans of care.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver’s license and proof of auto insurance
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred
