Conduct thorough evaluations of physical abilities, medical history, and functional limitations while administering exercises to improve strength, flexibility, and range of motion. Educate patients and families on home exercises and self-management techniques, then collaborate on discharge plans and coordinated care across departments. Monitor progress and document outcomes to adjust treatment as needed, while assisting with orientation and training of new staff.
