Job Summary
Develop and implement individualized physical therapy plans of care based on physician orders and agency policies. Conduct comprehensive patient assessments and evaluations, including OASIS C evaluations, while communicating with physicians prior to initiating treatment. Provide skilled therapeutic treatments such as pain management, heat therapy, and exercise programs, then continuously assess patient responses to recommend modifications. Determine service scope and frequency based on acuity in consultation with the Clinical Manager. Develop discharge plans using interdisciplinary input and participate in team meetings to coordinate care. Travel daily throughout the service area to visit patients in homes or residential settings across Florida.
Required Qualifications
- Current, active Physical Therapist license issued by the Florida Board of Physical Therapy Practice and in good standing
- Strong knowledge of OASIS and home health regulations
- One year of physical therapy experience
- Strong communication and documentation skills
- Ability to work collaboratively as part of a healthcare team
- Commitment to patient-centered, safe, and compassionate care
- Safely lift, carry, push, or pull up to 50 pounds
- Assist with patient transfers and ambulation
- Tolerate prolonged periods of physical activity, including standing and walking
- Respond quickly in emergency situations
- Travel throughout the service area on a daily basis to visit patients/clients
- OSHA Category 1
- Must follow universal precautions, infection control procedures and personal protective equipment (PPE) are required
- This role requires a screening through the Care Provider Background Screening Clearinghouse
