Job Summary
Assess and diagnose patients' physical conditions to develop personalized treatment plans, then create and execute rehabilitation programs to improve functional deficits. Perform manual therapy, therapeutic exercises, and modalities such as ultrasound and electrical stimulation as needed. Educate patients and caregivers on pain management, fall risk safety, and injury prevention while monitoring progress to adjust treatment plans. Collaborate with doctors, nurses, and other therapists to ensure comprehensive care in a rural setting. Full-time role with a $45–$55 hourly wage, up to $7,500 bonus, and tuition assistance.
Required Qualifications
- Graduate from an Accredited Physical Therapy Program
- Bachelor's degree
- Current accredited MN Physical Therapist License
- Strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care
- Capability to perform the essential functions of the job, with or without reasonable accommodations
- Ability to communicate in English, both verbally and in writing
- Sufficient communication skills to perform the tasks required
