Job Summary
Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, and modify care plans while assisting patients with exercises and hands-on therapy to manage pain and increase mobility. Educate patients on their established goals and self-care strategies, and coach families on safely assisting with exercises. Work as part of a home health team including RNs, LPNs, CNAs, and OTs focused on the patient's plan of care. Maintain active PT licensure in Illinois and CPR certification while adhering to state and federal home health regulations.
Required Qualifications
- Graduate of an accredited Physical Therapist Program
- Active PT license in Illinois
- Minimum of 1 year of physical therapy experience
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgement
- Strong interpersonal skills
- Dexterity
- Compassion
