Physical Therapist (57406)

Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury. Work as part of a home health team focused on the patient’s plan of care and goals, observing movements, conducting assessments, documenting progress, and modifying the care plan as needed. Assist patients with exercises and hands-on therapy to manage pain and increase mobility. Educate patients on their plan of care, established goals, exercises and self-care, and coach family on the plan of care and safely assisting with exercises.
