Job Summary
Home Health Physical Therapist (PT) in Deltona, FL and surrounding areas. Provide one-on-one, home-based physical therapy focused on restoring strength, mobility, and functional ability. Work within a home health team (including RN, LPN, CNA, HHA, OT, SLP) to implement and adjust the plan of care, educate patients and families, and help patients regain independence through exercises and hands-on therapy. Requires active FL PT license, at least 1 year of PT experience (preferably home healthcare), and CPR certification. Emphasizes flexible assignments, autonomy, and patient-centered outcomes, with access to EMR, ongoing training, and opportunities for CEUs.
Required Qualifications
- Graduate of an accredited Physical Therapist Program and active PT license in Florida
- Minimum of 1 year of physical therapy experience, ideally in home healthcare
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgement, strong interpersonal skills, dexterity and compassion
