Job Summary
Plan, organize, and implement physical therapy services for PACE participants through in-person assessments, treatment program administration, and home safety evaluations. Collaborate with the interdisciplinary team to develop comprehensive care plans, document clinical services, and educate participants and families on home programs. Train staff on body mechanics, equipment use, and gait patterns while actively participating in daily team meetings and quality improvement projects. Provide high-quality care in centers, clinics, contracted facilities, and participants' homes to assist individuals in functioning at the most independent level possible.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver's license
- Proof of auto insurance
Desired Qualifications
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting
- 1 year of experience working with a frail or elderly population
