Job Summary
Evaluate therapy needs, create and revise treatment plans, and perform physical therapy procedures as ordered by physicians or documented on IEPs for children with congenital disabilities, physical illness, or injury. Collaborate with team members, family members, and speech-language pathologists to facilitate comprehensive treatment and serve as a supervisor and mentor to PTAs and student PTs. Participate in in-service education programs and deliver formal staff training. Record all evaluations, progress notes, consultations, and discharge summaries in patient charts while maintaining records per agency guidelines. Make recommendations for specialized program materials, supplies, and adaptive equipment.
Required Qualifications
- Graduate of a Physical Therapy curriculum from an accredited university or college
- New York State Licensed and Registered as a Physical Therapist
- Display thorough knowledge of the principles and practices of physical therapy
- Adhere to the standards of clinical practice established by the American Physical Therapy Association
- Responsible for the evaluation of therapy needs
- Participate in the creation of the treatment plan, revising as necessary
- Perform physical therapy procedures as ordered by physician or documented on IEP
- Communicate with OT/PT Manager as appropriate to evaluate patient needs
- Serve as a supervisor and mentor to PTAs as well as student PTs
- Participate in the delivery of in-service education programs
- Participate in formal staff training and formalized in-service training as appropriate
- Record on patient charts all appropriate occupational therapy evaluations, progress notes, consultations and discharge summary information
- Submit all notes, evaluations and required forms within agency guidelines
- Maintain such records and prepared such reports as requested by the agency
- Make recommendations for purchases of specialized program materials, supplies and adaptive equipment
- Knowledge of Microsoft office and other computer-based programs
- Vision, speech, tactical and hearing senses adequate to perform job requirements
- Physical actions may include but not be limited to; standing, sitting, bending, lifting, kneeling, squatting, reaching, crawling, running, twisting & turning
- Frequent sitting/ standing and when appropriate, floor sitting with pediatric patients
