Job Summary

Physical Therapist provides PT services to program participants under the supervision of the Center Manager. Performs clinical physical therapy evaluation and treatment for participants in acute, restorative, or maintenance levels across Day Center, Nursing Home, or in-home settings. Integrates the treatment plan into the Interdisciplinary Team plan of care and assists in ordering and maintaining durable medical equipment. Conducts in-person assessments and re-assessments, collaborates with the Interdisciplinary Team, and participates in care delivery and planning for participants. Responsibilities include coordinating with nursing staff, planning care with adaptive equipment, and supporting home- and facility-based therapy services.

Required Qualifications

  • Graduate from a physical therapy curriculum approved by the American Physical Therapy Association and is currently licensed as a Physical Therapist in state where practicing
  • Minimum of one-year experience working with a frail or elderly population required.
  • Must clear the background check.
  • Must have a valid driver’s license and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
