Job Summary
Physical Therapist provides rehabilitative therapy to patients with physical functioning disorders and wound care, following regulatory and clinical practice standards. Responsibilities include assessing patients, developing and implementing plans of care, utilizing standardized assessments and evidence-based interventions, documenting treatment progress, coordinating with the health team, and providing discharge summaries. Supervises Physical Therapist Assistants, students, and rehabilitation aides as needed. Must be a graduate of an accredited Physical Therapy program and hold current active state licensure; one year of LTAC or related setting experience is preferred. Knowledge of Medicare PPS, regulatory requirements, and effective communication within interdisciplinary teams is essential.
