Job Summary
Perform patient-centered evaluations to determine functional levels and establish therapeutic plans of care. Develop comprehensive therapy plans including treatment, diagnosis, duration, frequency, long-term goals, and discharge plans. Document observations, patient responses, treatment programs, progress, and notable condition changes. Coordinate care with the health care team and patient. Supervise physical therapy assistants per regulations. Provide teaching to patients, families, and caregivers regarding safety and functional status. Assist patients and families in procuring needed equipment for optimal functioning.
Required Qualifications
- Minimum of a degree from an accredited Physical Therapy program for RPT
- Licensed as a Physical Therapist in the Commonwealth of Massachusetts
- Prefer a minimum of one year of experience in a therapist capacity
Desired Qualifications
- Preferred experience in skilled nursing facility setting or rehab hospital
