Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities, performing evaluations, developing individualized plans of care, and delivering evidence-based treatment. Document sessions within FOX's EMR system and collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff to promote safety and aging in place, while incorporating telehealth and virtual care when appropriate to extend access. Manage your own treatment schedule to support mobility, independence, and functional outcomes for clients across Fort Myers, FL.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Performing CPR
- Providing moderate to maximum assistance with patient transfers and bed mobility
- Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
- Driving to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Wearing PPE when required
- Must be cleared through the Florida Background Check Clearinghouse
