Job Summary
Evaluates, plans, and administers physical therapy in rehabilitation settings; establishes individualized treatment goals, provides therapies and regularly reassesses patient progress with accurate documentation of evaluations, treatment plans, progress notes, and billing. Educates patients and families on treatment plans and post-treatment care; collaborates with referring physicians and interdisciplinary teams; maintains safety, infection control, and adherence to policies. Requires Doctorate in Physical Therapy (DPT) and PT license in the state of employment, 0-2 years of experience, and BCLS within 30 days of hire. This is a full-time role with daytime hours focusing on diagnostic evaluations, treatment planning, and patient care continuity.
Required Qualifications
- Doctorate in Physical Therapy (DPT) required
- 0-2 years of experience in a physical therapy setting required
- PT - Physical Therapist license in the state of employment required
- BCLS - Basic Life Support within 30 days of hire required
