Job Summary
Conduct one-on-one rehabilitation care in patients' homes, assisted living facilities, or nursing homes to help individuals regain strength, restore mobility, and return to meaningful daily activities. Assess each patient's functional abilities and home environment, develop individualized treatment plans, and guide them through targeted interventions that improve mobility, balance, strength, and safety. Work closely with an interdisciplinary care team to educate and empower patients and caregivers while supporting measurable progress toward recovery goals. This full-time role involves daily travel within the Saginaw, MI service area and requires a current state license, Master's or doctoral degree, and at least one year of experience.
Required Qualifications
- Master's or doctoral degree from an accredited Physical Therapy program
- Current state license as a Physical Therapist
- Minimum 1 year of experience as a licensed Physical Therapist
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
- Willingness to travel daily for home visits within service area
Desired Qualifications
- Home health experience
- OASIS certification or willingness to obtain
- Experience with Medicare home health documentation
