Job Summary
Develop direct patient care by evaluating functional needs, adjusting care plans, and implementing therapy protocols based on thorough assessments. Design and execute individualized treatment plans while directing support staff to achieve functional goals. Document evaluations, treatment objectives, and prescriptions regularly to maintain compliance with defined standards of care. Promote the organization's philosophy and administrative policies to ensure quality outcomes across the home care setting. This physically demanding role involves lifting up to 50 pounds and requires full-time status with potential irregular hours and call rotations. Candidates must hold a current APTA-approved license, possess at least one year of healthcare experience, and be willing to travel 50% of the time.
Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of work
- Minimum one (1) year of experience in a healthcare facility
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Must be able to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
