Physical Rehab Certified Nursing Assistant-PRN Days/Nights

Conduct patient observation rounds every 15 minutes, assist with transfers, lifting, and turning, and obtain vital signs with accurate documentation. Ensure patient rooms are clean, feed, bathe, dress, and groom patients while maintaining dignity and privacy. Apply de-escalation techniques to manage distress, monitor behavioral improvements, and report changes to the nursing team. Collaborate with therapists and physicians to reinforce treatment goals and coordinate transportation as required. Adhere to work schedules and facility policies while communicating clearly with patients, families, and staff.
